Qualification Requirements

• Valid business license Bidder has to submit valid business license indicating the stream of business in which the bidder is engaged, in accordance with ITB Clause 4.6(b)(i); Reference: 33.2(c)
• Bid Submission Sheet Form Bidder has to provide in the Bid Submission Sheet Form the Statement attesting the origin of the Services offered Reference: 33.4(a)
• Valid tax clearance certificate Bidder has to submit a valid tax clearance certificate issued by the tax authority, in accordance with ITB Clause 4.6(b)(iii); Reference: 33.2(f)
• VAT registration certificate Bidder has to submit VAT registration certificate issued by the tax authority in accordance with ITB Clause 4. 6(b)(ii) Reference: 33.2(e)

የልብህክምናአገልግሎት 1. በልብ ህክምና ስፔሻላይዝድ ያደረገና በሰብስፔሻሊስት ደረጃ ባለ ሙያዎች ያሉት እና መሟላት ያለበት ህጋዊየህክምናፍቃድያለው 2. ከታች የተጠቀሱትን ምርመራዎች መስራት የሚችል እንዲሁም የአዋቂና የህጻናት ቀዶ ህክምና(pediatric cardiac surgery) መስራት የሚችል የተሟላ የፅኑ ህሙማን ክትትል (ICU)ያለው 3. የህክምና ተቋሙን ለማስጎብኘት ፍቃደኛ የሆነ 4. የታካሚ የህክምና ወጤትና ዶክመንት በግዜ ማሳወቅና በጥንቃቄ መያዝ የሚችል 5. ቀጣይነት ያለውና የማይቆራረጥ ሰረቪስ መስጠት የሚችል the procuring entity has the right to cancel the bid fully or partially.
