• Warranty Bidder has to submit Description of the organization of the warranty offered in accordance with the conditions laid down in GCC Clause 23; Reference: ITB 35.4
• Technical Specification + Technical Offer+ Compliance Sheet Bidder has failed to complete its Technical Specification + Technical Offer+ Compliance Sheet Form in accordance with technical specification presented as per template in Section 6, Statement of Requirements . Reference: ITB 35.4
• valid tax clearance certificate Bidder has to submit a valid tax clearance certificate issued by the tax authority in accordance with ITB Clause 4.6(b)(iii);  Reference: ITB 35.2
• Delivery and Completion Schedule Bidder has failed to submit dated Delivery and Completion Schedule presented as per template in Section 6, Statement of Requirements Reference: ITB 35.4
• valid business license Bidder has to submit valid business license indicating the stream of business in which the bidder is engaged in accordance with ITB Clause 4.6(b)(i); Reference: ITB 35.2
• VAT registration certificate Bidder has to submit VAT registration certificate issued by the tax authority (in case of contract value specified in BDS Clause 4.6(b)(ii) Reference: ITB 35.2

1.የግዥ ፈፃሚው አካል ስም የመከላከያ ጤና ዋና መምሪያ(Defence Health Main Department) ነው፡፡ ዋና መምሪያው ጨረታውን በከፊል ወይም ሙሉ ለሙሉ የመሠረዝ መብቱ የተጠበቀ ነው፡፡ 2.ለልብስ ስፌት አገልግሎት የሚውሉ ማሽኖችንና ለሰርቨር ሃይል ማቆየት አገልግሎት የሚውል ዩፒኤስ ግዥ ዋጋ ስትሞሉ በሲስተሙ ተያይዞ በቀረበው ስፔስፊኬሽን ተያይዞ በቀረበው መሠረት ጥራታቸውን የጠበቁ መሆን ይኖርባቸዋል፡፡ 3.በቀረበው ስፔስፊኬሽን መሠረት በሲስተሙ አያይዛችሁ በምትልኩት ስፔስፊኬሽን መሠረት ተገምግሞ ዝቅተኛ ዋጋ በማቅረባችሁ አሸናፊ የሆናችሁ ድርጅቶች ማቴሪያሎቹን ገቢ ከማድረጋችሁ በፊት በዋና መምሪያው ቴክኒካል ቲም ጥራቱ የሚረጋገጥ ይሆናል፡፡ 4.ተጫራች ድርጅቶች በሚያቀርቡት ቴክኒካል ዶክመንት ላይ የዋስትና እና የማቅረቢያ ጊዜ በግልፅ ማስቀመጥ ይኖርባችኋል፡፡
